Due to continued growth, we have an exciting opportunity for a hardworking and experienced Paramedic, to join Greenway Community Practice working with a supportive multi-disciplinary team in a high performing surgery in Northwest Bristol.

This is a varied role - assessing and treating patients, undertaking tests and investigations as well as supporting care planning and preventative treatment, with a particular focus on care homes and patients in own homes.

The successful candidate will be an experienced Paramedic with HCPC registration, educated to BSc level. Experience in Primary care is essential. You will have experience of managing a caseload, working autonomously within the community using enhanced clinical assessment and treatment skills, confident in making clinical judgements and managing risk.

The role is pivotal to improving the quality of care and operational efficiencies within the surgery, and so requires motivation and passion to deliver excellent service within general practice. You should have excellent interpersonal and communication skills and the ability to build effective relationships with patients, their carers and other agencies.

Your role will include assessment, triage, and referral of patients, undertaking tests and investigations, supporting development of care plans. You will support the team in delivering high quality services to patients in care homes and support home visiting. You will support diagnosis and treatment of acute conditions, implementing and evaluating treatment and care plans. You will see those who present with a wide range of minor illness and who have urgent and same day requests.

The Duty team offers urgent appointments for patients who feel they can’t wait for a routine appointment or who need an urgent home visit. You will work closely with other clinicians (GPs & Pharmacists) to deliver this service.

You will be responsible for providing telephone and face to face consultations for patients presenting, unscheduled, with a wide range of minor injuries and minor illness needs.

You will be expected to:

  • Carry out home visits to support the Duty Doctor
  • Independently decide on and carry out treatment working under the remit of local PGDs.
  • Assess and evaluate, with patients, the effectiveness of the treatment and care provided.
  • Refer patients for investigations and/or the appropriate specialist where appropriate.
  • Ensure the provision of continuity of care including follow-up visits.
  • Take a holistic approach to patient care, bringing together their medical, social and psychological needs.
  • Empower patients to take responsibility for managing their own care and treatment where it is safe and appropriate to do so.

For those patients at risk of hospital admission, for example, in a care home or with complex needs, they have a care plan which is communicated to the out of hours GP service. The Paramedic is responsible for creating and updating care plans for new patients when involved in their care.

For this part time role, we offer a competitive salary, NHS pension and 31 days annual leave pro rata. You will have access to regular clinical supervision.

Location

Greenway Community Practice is located in Southmead, a northern suburb and council ward of Bristol. Filton (in South Gloucestershire) and the Bristol suburbs of Monks Park, Horfield, Henleaze and Westbury on Trym are on its boundaries.

The area is well served with a range of local parks and sporting facilities. On the Southmead development trust site, where the practice is based, is a vibrant community gym and Café. A David Lloyd private gym is located next to our Practice and Horfield Sports Centre is a mile away.

The Practice

You would be part of a strong and experienced clinical team comprising of 5 partners, 3 salaried GP’s, 2 clinical pharmacists, 4 Nurses and 3 HCAs and a First Contact Physiotherapist
