Primary Care Network (PCN) changes
We are pleased to share that our practice is joining a new Primary Care Network (PCN) called ‘Phoenix’ from 1st April 2025.
- This collaboration is an exciting opportunity that will bring significant benefits to you and the practice.
- The member practices in this PCN include Gloucester Road Medical Centre and Horfield Health Centre.
Working together
By working together as part of a larger network, our practices will be able to coordinate services more effectively. The practices within this network are more aligned with our patient demographic, ensuring that services are closely suited to the needs of our patients.
Joining Phoenix PCN gives wider opportunities to collaborate on administrative responsibilities and streamline back-office functions. This will allow us to reduce duplication of effort, improve efficiency, and focus more on direct patient care. This will also allow the practices in the PCN to operate more efficiently at scale, making us all more resilient to external challenges and providing the stability needed to continue delivering high-quality care.
Although we are joining forces, we remain committed to maintaining the individual identities and values of each practice. Each of us brings something unique to the table, and this partnership is built on mutual respect for our distinct cultures and ways of working. The focus on collaboration is about amplifying our strengths, not compromising what makes each practice special.
