Greenway Community Practice is located in Southmead, a northern suburb and council ward of Bristol. Filton (in South Gloucestershire) and the Bristol suburbs of Monks Park, Horfield, Henleaze and Westbury on Trym are on its boundaries.

The area is well served with a range of local parks and sporting facilities. On the Southmead development trust site, where the practice is based, is a vibrant community gym and Café. A David Lloyd private gym is located next to our Practice and Horfield Sports Centre is a mile away.

 

The Practice

You would be part of a strong and experienced clinical team comprising of 5 GP partners, 5 salaried GP’s, 2 clinical pharmacists, 4 Nurses and 4 HCAs a Paramedic Practitioner and a First Contact Physiotherapist.

We are very proud to have been rated as ‘Outstanding’ by the Care Quality Commission and voted 4 times the ‘Best GP Practice in Bristol’ by our patients in the annual GP patient survey.

We are ranked 65 in the top 100 practices in England and have one of the best patient recommendation scores with 95% of patients recommending our service.

We are a popular training practice with an excellent staff retention rate. We engage in regular clinical meetings and invest time to interact with each other on a daily basis. There is also a mentorship scheme in place for newly qualified GPs.

Salaried GP - Permanent Contract

The Role

  • 6.25 Sessions per week [includes one paid admin session]
  • Personal list system with low Patient:GP ratio
  • Supported and encouraged to develop specialist interests.
  • On call duties on rotation [1 per week]
  • Minimal home visiting which is shared throughout the team.
  • EMIS web & Docman 10.
  • Contribute to a progressive, forward-thinking team.

Care Co-ordinator

Greenway Community Practice is seeking a compassionate, organised, and proactive Care Coordinator to join our team. This vital role supports patients in navigating their journey through the community, with a strong focus on vulnerable and complex patient groups, including regular liaison with local care homes and supporting with the seasonal vaccination programmes.

The post holder will need to adhere to the practice coding policy and will also support the multi-disciplinary team, providing administrative support as required, in line with the needs of the practice. The candidate will need to be an excellent communicator and be able to establish efficient monitoring and quality assurance systems.

The Role

Working collaboratively with GPs and other primary care professionals within the practice to proactively identify and support a specific cohort of patients in delivering personalised care.

Assist patients in using decision aids to prepare for shared decision-making conversations and contribute to the development of personalised care and support plans, aligned with best practice guidelines.

Support individuals in managing their health needs by responding to queries, arranging and managing appointments, and providing clear, high-quality written or verbal information to enable informed choices about their care. This may also involve helping patients access self-management education, training or employment opportunities, and supporting applications for appropriate benefits where eligible.

In addition, the role will involve assisting with the administration of care home ward rounds and supporting the coordination and delivery of seasonal vaccination campaigns.

  • 28.5 hrs per week across a minimum of 3 days
