Medication reviews for patients diagnosed with heart failure
From December 2025, our Clinical Pharmacists will be offering face-to-face medication review appointments for patients diagnosed with heart failure.
These 30-minute reviews are designed to help you understand your condition better, check how well your heart medicines are working, and make sure your treatment plan keeps you feeling as well as possible.
What is “heart failure”?
The term heart failure can sound worrying, but it doesn’t mean your heart has stopped working. It means your heart isn’t pumping blood as effectively as it should, which can cause symptoms such as:
- Tiredness or low energy
- Shortness of breath
- Swelling in your ankles, legs, or tummy
Many people live well with this condition with the right medicines, healthy habits, and regular check-ups.
What happens at the review?
During your appointment, our clinical pharmacist will:
- Discuss how you’ve been feeling and any symptoms you’ve noticed
- Review recent blood test results
- Review your current medicines and adjust doses if needed
- Offer advice on lifestyle, diet, and managing your condition day to day
Appointment bookings
These reviews will usually take place after your annual blood test appointment with our healthcare assistant (HCA). This helps us make sure your review is based on the most up-to-date results. The HCA’s will kindly book you into the clinic whilst you’re attending your annual blood test appointment with them.
