Confirm your address letter

 

You may recently have received a letter from NHS England/Primary Care Services England asking you to confirm your address details. Please be assured that this letter is genuine and has been sent as part of a national NHS address verification process.

The purpose of this is to ensure that all NHS organisations hold the correct contact details for you so that you continue to receive important information about your healthcare, appointments, vaccinations, and other NHS services.

It is very important that you respond within 30 days, even if your address details have not changed. If no response is received, the NHS will begin the process of removing you from our practice list to protect your personal information.

Being removed from the practice list can have a significant impact on your access to healthcare services and will mean that you need to re-register with the practice. Re-registration can be time consuming and may delay access to routine care and appointments.
