Living Well with COPD
Living Well with COPD is a pilot programme in Bristol, North Somerset and South Gloucestershire. It will support people living with chronic obstructive pulmonary disease (COPD) at home, to support self management.
It offers access to a specialist my mhealth web-based app, myCOPD, to help manage COPD at home.
myCOPD is a digital app for people living with COPD, designed to help better self-management. It includes guidance on inhaler technique with easy-to-follow inhaler videos, as well as expert support on how to manage COPD and much more from the comfort of your own home.
Extra support for people who also have a heart condition is also available on the app. myHeart brings the best digital support to those living with any of the following conditions:
- heart failure
- angina
- heart attack
- post PCI
- valve replacement
- valve repair
- coronary artery bypass graft Surgery (CABG)
- valvular heart disease.
Tailored for you, myHeart has a range of features such as, information to help you learn more about your condition and day to day management, walking and rehabilitation program with an activity diary to keep track of progress, activities to support stress reduction and anxiety management, and much more… myHeart is there to support you to manage your condition.
my mHealth is working with local voluntary sector groups and locality partnerships to support patients to use technology. Patients will be directed to local library services if they do not have suitable devices – for example smartphones.
The myCOPD app is currently only available in English. my mhealth is working with local voluntary sector groups to explore ways to support patients whose first language is not English to use the app.
my mhealth Digital Health Advisers can also support you to get the most from the app and they may reach out to offer support or can be contacted via email.
If you are offered myCOPD but do not want to take part, you can say no at any time for any reason, and you will not be contacted again.
